Military ID / Military Status Declaration
A separate document, also related to civic duties: the cédula militar (military ID) is the official document proving a citizen's military status, and it replaces the old "declaração de situação militar" for legal purposes. Military registration has been automatic since 2009 — there's no need to enrol in person.
It's requested through the BUD portal (Balcão Único de Documentos), run by the Directorate-General for National Defence Resources (DGRDN):
Military ID: bud.gov.pt/ddn/cedula.html
Military status declaration: bud.gov.pt/ddn/declaracao.html
